Eligibility requirements
You may be eligible for a grant if you were one of the following:
- a ward of the State of Queensland
- under guardianship of the State of Queensland
- a resident in a Queensland institution including one of the 159 institutions listed in the Forde Inquiry report.
You don’t need to be currently living in Queensland. If you meet the eligibility criteria, you can still register.
Confirm your proof of eligibility
To register for the Forde Foundation, we first need to confirm your eligibility.
To confirm eligibility, we must ask that you provide government records that state when and where you were in care as a child in Queensland, or resident in a Queensland Institution.
This may be:
- records from the Right to Information Unit, or Freedom of Information as it was once known
- letters or government documents, for example a copy of any correspondence referring to a redress application
- a Care History Summary report.
Please be assured that any personal information you provide, will be managed in a sensitive and confidential manner and in line with privacy requirements.

Complete our registration form
Once you have your proof of eligibility you can complete our online registration form.
Please make sure you have a copy of your proof of eligibility with you before you start. You will need to upload them with your registration.
You can phone or email us for help or advice in:
- accessing your records
- completing the form.
There is no cost to register, and you only have to register with the Forde Foundation once.
You can’t apply for a grant if you have not registered with the Foundation.
