Register with the Forde Foundation

 

The Forde Foundation needs to confirm your time in care before you can register with the Forde Foundation. The easiest way to do this is by requesting a Care History Summary Statement.

 

Care History Summary Statement


A Care History Summary Statement is a short document that lists the homes and institutions where you resided while in out-of-home care in Queensland.

The Department of Communities, Child Safety and Disability Services gathers this information from the records that are available to it.

The Statement will allow the Forde Foundation and other service providers to confirm you were in out-of-home care in Queensland during your childhood.

 

How to request for your Care History Summary Statement


1. Download the Care History Summary Statement Request Form by clicking here.

2. Complete the Care History Summary Statement Request Form.

3. Ask a Justice of the Peace (JP) or Commissioner for Declarations to certify a copy of your photo ID.

4. Post your Care History Summary Statement Request Form and certified photo ID to the Forde Foundation.

5. The Department of Communities, Child Safety and Disability Services will send you 2 copies of your Care History Summary Statement.  However, be aware that it could take up to 30 business days to process your request. 

 

How to register with the Forde Foundation


1. Download the Forde Foundation Registration form by clicking here.

2. Complete the Forde Foundation Registration form and attach one copy of your Care History Summary Statement.

3. Post your Care History Summary Statement and Forde Foundation Registration form to the Forde Foundation, GPO Box 806, Brisbane, Qld, 4001.
